Meet the staff
Dr. Anita M. Larrow, ND
Naturopathic Doctor
Owner / Proprietor
Dr. Anita M. Larrow received a BA in Biology and Chemistry from Bluffton University, in Bluffton, Ohio. She then graduated, with Honors, from the Southwest College of Naturopathic Medicine and Health Science (SCNM) (now named Sonoran University of Health Sciences) in Tempe, Arizona. Throughout her medical training at SCNM, Dr. Larrow worked with MDs, DOs, and NDs in Arizona, California, and Ohio thereby allowing her to understand the wide range of treatment options presented to patients. She challenged herself to appreciate the different medical structures while not summarily judging one better than another. Instead, she understands each medical structures place in the treatment of the patient. Dr. Larrow believes that the purpose of being a doctor is to help the patient, and that is the bottom line.
Chris Ibarguen
Office Manager
Chris's background contains retail, customer service, and support. With years of customer service experience in both office environments and customer facing business, he's learned to properly help patients, customers, and other businesses with their needs. He has an Associates of Automotive Technology from Universal Technical Institute (UTI) in Avondale, Arizona. There he learned both automotive technology and customer relations, later becoming a shop foreman at a dealership in Napa, California. Chris understands what it means to support, and understand, patient and customer needs. But also how to help and encourage his colleagues to create a positive working environment.

Patient Intake Form (Interactive)
Before your first visit, please fill out this intake form as best possible. It is an Adobe PDF form that can be saved during and after inputting information. If you have questions about any of the items on it, Dr. Larrow will go over these with you during your initial consultation.

You can File > Save (or Save-as) at any time and come back to the form to finish later it if needed
Please remember to also File > Save (or Save-as) when done then email or fax (or print and mail) the completed form back to info @ drlarrownd.com or 1/707/948/6037, respectively
Pediatric Intake Form
This form is to be filled out by the child's parent before their first visit with Dr. Larrow. As with the normal intake form, don't worry if you have questions about any of the form items as Dr. Larrow will go over them with you.

Policy Information
The following forms are our policy documents. They contain doctor and patient responsibility, HIPAA, privacy information, etc. They will be given to you at your first visit so you do not need to print them out. However, please feel free to review them at your leisure here.
